Education and Management Are Key to Controlling Diabetes

The more you know about diabetes, the better you’ll be able to manage its effects on your life. Through both education and treatment, we are here to provide support for you.

Diabetes and nutrition services are provided to people of all ages and including:

  • Diabetes education provided by Certified Diabetes Care and Education Specialists including nurse/dietitian team for patients newly diagnosed with diabetes or those who have had diabetes for a long time. This can include patients managing with diet and exercise treatment and/or on medications treatments for diabetes.
  • Advanced Practice Professionals provide overall comprehensive diabetes assessment discuss treatment options for managing your diabetes. This may include medication management and/or diabetes complication management.
  • Basic and in-depth education programs are available to teach diabetes self-management and ways to prevent diabetes complications so you can confidently take control of your health.
  • Individualized dietary treatment plans, created by Certified Diabetes Care and Education Specialists to include meal planning, medications, exercising, testing blood sugar and coping with a different lifestyle
  • Lifestyle change planning, coaching and education for people diagnosed with prediabetes, a condition in which blood sugar levels are higher than normal, but not high enough to be considered diabetes
  • Consultation and follow up with pregnant women diagnosed with gestational diabetes or those with Type 1 and Type 2 diabetes and now pregnant are appropriately managed to ensure a healthy pregnancy and a healthy baby
  • Education and management of advanced diabetes technologies including professional continuous blood glucose monitoring, personal continuous blood glucose monitoring, insulin pump technology, and insulin smart pen technology.
  • Additional support services including assisting with applying for drug assistance (to lower cost of medications), finding additional low resources, and connecting patients with diabetes to community support services.
