By choosing to schedule your curbside COVID-19 test online, you are agreeing to the General Consent for Admission.
For VA patients: If you receive benefits through the VA, please schedule your COVID test with the VA. Prior authorization is required from the VA for COVID testing done at CentraCare or Carris.

Symptomatic
I have symptoms of COVID-19.

Asymptomatic
I do not have symptoms of COVID-19, but I want to get tested for another
reason. Or, I do not have symptoms but I was exposed to someone who tested
positive for COVID-19.
If you have a known exposure to someone with COVID-19, it is best to be tested
at least 5 days after you were exposed.

Pre-Procedure
I am a patient who has been directed to have COVID testing prior to my
scheduled outpatient procedure or surgery; or prior to my scheduled OB
induction or C-Section; or I am an OB patient at greater than 38 weeks
gestation.

Symptoms of COVID-19
Symptoms of COVID-19 include:
- Fever or chills
- Cough
- Shortness of breath or difficulty breathing
- Fatigue
- Muscle or body aches
- Headache
- New loss of taste or smell
- Sore throat
- Congestion or runny nose
- Nausea or vomiting
- Diarrhea
